Become a Family Heart Foundation Advocate for Awareness
Raise awareness and change lives by volunteering with the Family Heart Foundation.
Familial Hypercholesterolemia (FH), Homozygous Familial Hypercholesterolemia (HoFH), elevated Lipoprotein(a) (Lp(a)) and other polygenic lipid disorders are all under-diagnosed and under-treated, leaving people at risk for early, aggressive cardiovascular disease. If you want to be a part of making sure that families with inherited risk for heart disease, stroke, and premature death have a chance to live longer, healthier lives, then please join us in the fight.
The Family Heart Foundation is always looking for motivated individuals to become Family Heart Foundation Advocates for Awareness. You can apply by completing the form here. A member of the Family Heart Foundation Team will follow up with you to set up a phone interview and tell you more about the program. Participants are selected based on the strength of their application, including relevant life experiences, volunteer history, and/or recommendations.
The Family Heart Foundation accepts applications throughout the year. We hold an annual Family Heart Foundation Advocates for Awareness Training Workshop. Attendees learn more about FH, elevated Lp(a), and polygenic lipid disorders from medical experts and are trained to tell their personal stories.

Who may apply to become an Family Heart Foundation Advocate?
Individuals who have Familial Hypercholesterolemia (FH), high Lp(a), or polygenic lipid disorders and their family members and who live in the United States.

About the Family Heart Foundation Advocates for Awareness Program
The Advocates for Awareness program is part of the Family Heart Foundation’s Community in Action activities. This incredible network of trained volunteers have stepped up to share their stories to raise awareness, to support their peers who are newly diagnosed, and to make sure the Family Heart Foundation continues to prioritize what is most important to the people we serve—individuals with FH, HoFH, elevated Lp(a), and polygenic lipid disorders and their families.
Family Heart Foundation Advocates support the mission of the Family Heart Foundation by:
- Raising awareness by sharing their story.
- Fundraising to support the Family Heart Foundation’s work.
- Offering Peer Support to others.
- Advising the Family Heart Foundation and our research partners.
Family Heart Foundation Advocates Training Workshop
Each year we hold an Advocates for Awareness Training Workshop to train new Advocates. This is your opportunity to meet other volunteers and the Family Heart Foundation Team, receive a comprehensive orientation to the Foundation, learn the medical aspects of FH, HoFH, elevated Lp(a) and cardiovascular disease from lipid specialists, and participate in one-on-one coaching on how to tell your personal story in a compelling manner. This in-depth training experience is intended to prepare you to be an effective Advocate for Awareness.
