
The Flag, Identify, Network, DeliverTM initiative is an innovative program designed to close the gaps in care for individuals living with familial hypercholesterolemia (FH) and/or elevated lipoprotein(a), also known as Lp(a).
This comprehensive program uses the real-world application of artificial intelligence, implementation science, and quality improvement science to help healthcare systems screen their populations for individuals at high risk for having FH and/or elevated Lp(a).
Once identified and diagnosed, individuals and their health care teams are offered a network of supportive resources from the Family Heart Foundation.
