Tribute and Legacy/Planned Giving

Making a gift in honor of someone or making provisions for the Family Heart Foundation in your will or trust are ways to have a lasting impact for people with FH and high Lp(a).

Go to the Family Heart Foundation donation form. Select the amount that you would like to gift.

Check “Dedicate my donation in honor or in memory of someone.”

Select “In honor of…” or “In memory of…” and complete the form.

By making provisions for the Family Heart Foundation in your will or trust, you can have a lasting impact for people with FH. Making a bequest can be as simple as adding the following language to your estate planning documents:

I give [ __ percent of my estate, or description of asset, or __ dollars] to the Family Heart Foundation, a 501(c)(3) nonprofit organization located at 680 East Colorado Boulevard, Suite 180–2005, Pasadena, CA 91101, Tax ID# 45–4597425, for its general use.
